Oregon's largest Coordinated Care Organization, serving 400,000+ OHP members across the Portland Metro tri-county area. The default CCO recommendation for trans clients on OHP because it includes OHSU, Prism Health, Multnomah County Health Centers, Planned Parenthood Columbia Willamette, and Outside In in network. Embeds Traditional Health Workers (community health workers, doulas, peer specialists) in clinics. Administers NEMT for medical transportation via Ride to Care.
