Prism Health

Primary care clinic of Cascade AIDS Project, developed with input from trans women of color. Provides gender-affirming care including hormone therapy initiation and management, surgery referrals, WPATH letter coordination, gender-affirming preventive care (pelvic exams, chest/breast health, prostate exams), and integrated mental health and substance use counseling. Trauma-informed, patient-directed care with gender-neutral restrooms and telehealth available statewide. Serves ages 15+. Prism Health has two Portland clinics: 2236 SE Belmont St (97214) and 15 N Morris St (97227).
