This is the Social Needs Request Form for members of Health Share of Oregon, the Oregon Health Plan (Medicaid) coordinated care plan for the Portland tri-county area, administered with CareOregon. Current members can apply online for help with rent, utilities, storage, and other social needs, including the Health-Related Social Needs (HRSN) housing benefit, which can cover up to 6 months of rent for people who qualify. You must be a current member, be unable to get the item another way, and provide documentation such as a lease or utility bill. Important: if you already have a court-ordered eviction notice, or a utility shut-off or disconnection, do not use this form. Call 211info at 503-468-5375 instead. Requests are reviewed case by case, so allow 7 to 10 business days.
